Renal medicine is one of the most interesting, exciting, challenging, and rewarding medical specialties in which to pursue a medical career. The experience available is very broad. It encompasses the management of often very sick patients with acute kidney injury which includes a substantial overlap with intensive care medicine; chronic kidney disease which provides an opportunity of chronic disease management in both pre-dialysis and dialysis settings; as well as renal transplantation with the chance to restore patients back to good health and a good quality of life. Renal medicine is one of the few remaining hospital based specialities that provides genuine long term follow up and general medical care to a significant number of patients with diverse and interesting underlying disease processes.
The majority of our trainees wish to dual accredit in renal and general medicine. This takes 5 years following entry at ST3 level. Single accreditation in renal medicine only takes 4 years following entry at ST3 level. Each unit provides experience in all aspects of nephrology, dialysis and long term renal transplant follow up. Experience in acute renal transplantation is provided at Nottingham City and Leicester General Hospitals. Therefore, all trainees will undertake some of their training in Leicester or Nottingham to ensure acute renal transplantation experience.

Following appointment to the Training Programme trainees would be offered the choice of four 3 year general medicine / nephrology rotations aimed at providing a sound grounding in general medicine and nephrology, dialysis and transplantation. These rotations are grouped into geographical clusters to minimise travelling and this gives trainees the advantage of knowing which hospitals they will be based in for the first 3 years of their training. During the final 2 years of your training we have designed a series of themed general medicine / nephrology rotations which will allow trainees to focus their final year of training towards a specific area they are particularly interested in such as dialysis, transplantation, AKI-HDU or general nephrology.
There are excellent training opportunities throughout the region. Trainees will have the opportunity to develop competency in renal biopsies, tunnelled dialysis catheter insertion and placement of peritoneal dialysis catheters. There is a well established educational programme for both general medicine and renal medicine. The renal medicine training programme consists of a series of 8 training days over the course of a year. We aim to cover the whole curriculum over a three year period and trainees are released from their units to attend these sessions. The training sessions are held in a venue away from the main hospital sites so trainees are guaranteed to have a bleep-free teaching experience.
In addition we organise a yearly national training course with colleagues in the North West specifically aimed at trainees undertaking the renal specialty certificate examination. Feedback from attendees has been universally good to excellent.
The East Midlands has internationally renowed research groups in renal laboratory-based, clinical, educational and epidemiological science. Out of programme experience is encouraged for all trainees. With opportunities in Leicester, Derby and Nottingham there are many projects to choose from. The majority of our trainees elect to undertake some form of out of programme experience; the majority completing higher degrees during this period of time.
We are flexible in our approach to training and we have an established track record in supporting trainees who wish to train at less than full time. Trainees must work a minimum of 5 sessions and the overall length of time in training is extended pro-rata, depending on the number of sessions worked. There is a speciality education committee for renal medicine within the East Midlands which has responsibility for overseeing training within the region. This has representatives from each of the hospitals on the rotation, the training programme and associate training programme director and trainee representatives. The committee meets quarterly.
We feel that training within the East Midlands region offers a wide variety of opportunities for trainees to develop not only the core clinical skills necessary for life as a renal physician, but also the additional skills which demonstrate ‘added value’ and make our trainees highly competitive when applying for consultant posts. Our record of trainees getting appointed to consultant posts of their choice is excellent.
